In this case study, we explore how the Amedisys Dementia Care Connect+ helps improve end-of-life care for patients with dementia by reducing pain, agitation, medication burden and unnecessary interventions and hospitalizations. Through specialized hospice support, patients experience enhanced comfort and quality of life while partner clinicians and caregivers gain guidance for complex care decisions.
